What Is a HIFU Facelift?

A HIFU facelift is a non-surgical method that uses high-intensity focused ultrasound (HIFU) energy to heat the deeper layers of the skin and help sagging tissue firm up. It needs no needles, incisions or anesthesia.

The ultrasound energy passes through the skin surface without harming it and concentrates at a set depth underneath. One of the layers it reaches is the SMAS (superficial musculoaponeurotic system), the same supporting layer a surgeon works on during a surgical facelift. The controlled heat there causes collagen fibers to contract and starts the skin’s repair process, during which new collagen and elastin are produced. Over the following weeks, the skin looks tighter and more lifted. At our clinic in Istanbul, this treatment is performed with LIFU technology, which delivers the energy in lines.

A HIFU facelift is usually a good fit for:

  • Early signs of sagging: Mild laxity, which often becomes noticeable from the thirties onward.
  • People who prefer to avoid surgery: Those for whom a surgical facelift is too early or who are hesitant about an operation.
  • People with little time for recovery: There is no need to take a break from daily life afterward.

With advanced sagging and clear excess skin, HIFU alone may not be enough.

HIFU vs LIFU: What Is the Difference?

LIFU (linear focused ultrasound) works on the same principle as HIFU but delivers the energy differently. Classic HIFU devices send energy as separate dots, while LIFU delivers it continuously along a line. In practice, this means:

  • Wider coverage: Dot-shaped pulses can leave gaps; linear pulses cover the tissue more evenly.
  • Shorter sessions: A full-face treatment usually takes 15–20 minutes.
  • Comfort: Because the energy is not concentrated on a single point, most people find the sensation milder.
  • A fat-reduction mode: A deeper LIFU mode can help reduce fat in areas such as the double chin.

The goal of both is the same: tightening tissue without surgery. Which depths and modes are used depends on the degree of sagging and is decided at your consultation.

Although it is sometimes called a “HIFU facial,” this is not a facial. The energy reaches supporting tissues well below the surface, so choosing the settings is a medical decision.

How Is a HIFU / LIFU Facelift Done?

Treatment begins with an examination and facial analysis, in which the doctor assesses the degree of sagging, skin thickness and fat distribution.

  1. The areas to be treated are marked.
  2. Transducers working at different depths, such as 1.5 mm, 3 mm, 4.5 mm and 6 mm, are chosen according to the degree of sagging.
  3. A cool gel is applied so the ultrasound waves pass into the skin.
  4. The handpiece is moved over the skin, delivering energy to the target depth.
  5. The gel is removed, and moisturizer and sunscreen are applied if needed.

The skin may look slightly pink afterward, which usually fades quickly. Bruising, crusting or wounds are not expected, and you can return to work the same day. Mild tenderness or brief swelling may occur and usually settles on its own.

In the first few days, use a high-SPF sunscreen regularly, avoid saunas, steam rooms and intense exercise, and pause peels and irritating products. Makeup can usually be worn the same day, or after a few hours if your skin feels sensitive.

What Areas Can HIFU Treat?

  • Jawline and facial contour: Helps define a softened jawline.
  • Brow lift: A slight lift of the brow tail opens up the eyes.
  • Double chin: Fat under the chin is reduced and loose skin tightened.
  • Neck: Aims to firm loosened neck skin.
  • Body: Can be used for laxity on the inner arms, above the knees and on the abdomen.

HIFU tightens tissue, but it does not replace lost volume. If the cheekbones or temples look hollow, it can be combined with a liquid facelift. For full cheeks, Agassi cheek slimming may be more suitable, and for marked sagging, methods that physically support the tissue, such as French lifting, come to the fore.

When Will You See Results and How Long Does HIFU Last?

Results appear in two phases. A slight tightening may be noticed right after treatment as collagen fibers contract. The main result develops as new collagen forms and usually becomes clear at two to three months.

Results typically last 12–18 months, depending on your age, skin structure and lifestyle. Because aging continues, repeat treatments can be planned at intervals; the timing is decided at a follow-up appointment, and there is no need to wait for the effect to wear off completely. Sun protection and not smoking help maintain the result.

To improve skin surface quality as well, treatments such as Golden Needle can be added to the same plan. In Dr. Sevgi Ekiyor’s Facial Architecture (Yüz Mimarisi®) approach, HIFU is one of the treatments planned in harmony with the proportions of the face.

Frequently Asked Questions

Popular Questions

Does HIFU hurt?

It usually does not require anesthesia. You may feel a mild tingling or warmth from within during treatment. Because the energy is spread along a line, most people find LIFU more comfortable than classic HIFU.

For most people, one session is enough. With more advanced sagging, a second session may be planned about six months later, based on the doctor’s assessment.

In suitable candidates, usually people with mild to moderate sagging, HIFU tightens and lifts the skin gradually as new collagen forms. It does not replace surgery for advanced sagging with clear excess skin. Results vary from person to person.

Settings are chosen for each person. In areas where fat loss is not wanted, only tightening modes are used; the fat-reduction mode is reserved for areas such as the double chin. When planned correctly, the aim is to lift and firm the face, not to hollow it.

It is not performed during pregnancy or breastfeeding, in people with a pacemaker or a metal implant in the treatment area, or in those with an active skin infection or a serious systemic illness.
